Southern Maine Agency on Aging has an opening for a part-time, 20 hours per week, Social Worker at our Adult Day Center. This role will work within our Family Caregiver Support program, with a focus on the Day Center. As an integral part of our team, the Social Worker will be responsible for effective communication with referral sources, admissions paperwork, tours, assessments, and the development of person-centered care plans for our members. The Social Worker will support family caregivers by providing information and referrals, education, training, options, and benefits counseling. This position will work closely in collaboration with the Adult Day Center Coordinator, and the Registered Nurse to offer educational opportunities to staff and family caregivers.
Candidate should be comfortable working remotely but also be able to report to our Biddeford, Maine office 2 – 3 times a week
Education/Knowledge Requirements:
Bachelor’s Degree in Social Work, Gerontology, or related field (preferred)
Social Work License (preferred)
Critical thinking skills such as analytical and problem-solving skills to assess and evaluate situations and needs to resolve issues of varied levels of complexity.
Excellent writing, communication, listening and interpersonal skills as the Social Worker interacts with staff, consumers, family members, community service providers and referral sources.
At least two years of experience in working with older people, adults with disabilities and caregivers with demonstrated knowledge of community resources and benefits.
At least two years of experience working with people who have been diagnosed with dementia.
